Right now, we’re looking for a case manager to join our growing team.


In this role, you’ll be responsible for working with our clients and their support workers to case-manage the specific needs of each client in our supported living homes. You’ll ideally have a background with NDIS and ideally have experience as a team leader or come from residential homes management.


Your role will include meeting with key stakeholders, including working with health providers and government bodies, as well as managing risk and writing up individualised management plans. What we’re looking for is someone who has a great personality, is comfortable dealing with often complex situations, and is passionate in delivering exceptional service to their participants.


What you’ll need in this job
  • Solid experience with disability care and sound experience dealing with NDIS

  • Administrative experience, and be comfortable with producing quality (and legally fileable) paperwork

  • Drivers’ license, as well as a reliable and fully-insured vehicle

  • Working with Vulnerable Persons check and able to pass a Police Check

  • Amazing verbal communication skills
